Government guidelines states that all consultations should be done online, so we’d really appreciate it if you could take the time to fill this form in after you book an appointment with us so we receive it prior to your treatment.
It’s the exact same form you would fill in on paper when you attend an appointment but now online so that we can be as contactless as possible to ensure your safety.
As an aromatherapist, I’m aware that some oils should be avoided with certain conditions. We ask during our online consultation for you to disclose any health problems which may prevent a treatment from taking place/an adaptation to be made. If you take regular medication, please let us know at the appointment, as sometimes oils can interact with them. We just want to ensure that you are catered for and safe during your appointment with us.

From the 24th September, we are required by law to display our QR code which has been provided via gov.uk to allow our clients to check in with the new NHS COVID-19 app. This will be our new approach to Track and Trace, however, if you do not have a compatible phone or would prefer not to use the app, we can provide an online form for you to fill in the day before your appointment.
